CAMAGÜEY.- This time we helped ourselves with two health professionals to address a controversial issue, such as Cosmetology and its relationship with Dermatology.

We spoke with Dr. Beatriz Mantecón Fernández, second-degree specialist in Dermatology, Master in Infectology and Tropical Medicine, Assistant Professor and Coordinator of the specialty of Dermatology in the province of Camagüey, who maintains her consultation for this purpose at the Manuel Ascunce Domenech University Hospital.

She assured that beauty, that which everyone makes their own in their own way, still means health and that skin and hair conditions can cause dissatisfaction in people, to the point that it leads to depression in many cases. Hence the importance of this type of consultation.

“It is the state of bio-psycho-social balance. Dermatology is vital in aesthetic medicine and in the training of personnel, both from the ethical point of view and in the responsibility of the professional, and thus avoid union intrusion and those who perform procedures without the knowledge, which can lead to complications " .

—How do you deal with some skin conditions?

—We exfoliate the skin, renew it, especially in cases that suffer from acne and others, and we include mesotherapy, in particular with plasma, we apply the drug to the diseased site intradermally, that is, below the epidermis, and to achieve what we want, it is done through micropuncture that can be manual, assisted (pistol), and the most innovative way through the Dermashine.

- What is mesotherapy and in what conditions is it used?

- Mesotherapy by the mere fact of injecting stimulates the tissues and it is used in alopecia because it activates the blood supply and nourishes the hair, reaches the collagen tissue in general, tries to regenerate it and also serves to eliminate scars, acne, stretch marks, and cellulite.

—In these types of consultations, what is injected through this mesotherapy?

—Plasma rich in platelets and vitamins. The plasma is autologous and harmless. It is from the same person that is treated.

“We obtain it from the blood, it is centrifuged, after anticoagulation, and remains as a supernatant and contains five times more platelets than circulating platelets, a procedure that is used in various conditions, among which is alopecia again, the photo aging, acne, scars, just to mention a few.

"This allows the skin to be nourished, it is smoother, luscious and illuminated as some call it, and another novelty is that it is used in vitiligo with partial results, specifically when it is with few lesions and of recent appearance".

—Do skin conditions have to do with Genetics?

—60% of the factors that influence aging are genetic in nature. For this reason, the future of cosmetics implies personalized medicine, hence there are studies based on DNA and cosmetics.

—How do you get to this consultation?

—Referred by other specialists, especially in Dermatology, although family doctors or others are authorized to send them when they notice some of these problems. They know our work that is already five years old.

—If we are different and genetics determines, why do we apply the same creams?

"Indeed, genetics has revolutionized cosmetology." It also influences nutrition as an internal process up to the cells. Although we do not have it yet, there are already genetic tests to know what product each person needs, because the skin is capable of nourishing itself from the outside, that is the future.

As we learned of the presence of * Dra. Martha Janneth Tobar Bonilla, specialist in Aesthetic Medicine and Health Management, Master in Homotoxicology, resident in Dermatology at the “Manuel Ascunce”, international lecturer at Dermashine Balance, Closter Pharma Laboratory, Bogotá, Colombia, we wanted to complete certain criteria with the use this proceed here.

She announced that the aforementioned Laboratory is a distributor of medicines and medical equipment, not only those related to aesthetics, and brought from her country the Dermashine, a Korean equipment that in Latin America is distributed by the said laboratory, while in Europe it began to be sold around 2012, and noted:

"It is a very new technique worldwide, it consists of a digital injection system with needles or microneedles by aspiration because it has a module so that the skin does not move, fix it and then there is less pain for the patient, although it has the pedal also used on the scalp and thus the hair does not become an obstacle, but it is always the choice of the doctor, the characteristics of the patient and the quality of the skin.

“Being digital it is programmed. Before the teams opened channels, they punctured you and you placed the product, with this it is possible to introduce the needle and inject the product directly into the dermis. There is no other in the market worldwide that does the same, at least so far.

“An agreement was reached with the laboratory to sponsor the thesis, so they gave me the equipment, a number of needles, and hyaluronic acid, and here we are in Camagüey. This is a country that is in the tropics and people do not protect themselves adequately, and photodamage is a condition caused by solar rays and almost everyone in Cuba has it ”.

—How many patients are under study?

—In the beginning, 40. We start with the platelet-rich plasma of each patient, and after centrifugation in the consultation, the appropriate one is selected, and it is something that had already been done, as Dr. Beatriz said; now what is new is that it is combined with hyaluronic acid and the equipment in question.

"Today there is a kit that brings that acid and it only remains to associate it with platelet-rich plasma and that is what we are doing in Cuba, in the same way, without the need for those kits that are very expensive and the result is very good. because we act at the level of the epidermis and dermis.

"The microneedles have the appropriate magnitude to reach the dermis directly and we know this because you see that the patient is bleeding and indicates that the exact place was reached."

One of the patients receives treatment with platelet-rich plasma because she suffers from acne and is applied by Dr. Beatriz.

—What do you call photodamage?

—It's the pigmentation, the wrinkles, the dryness of the skin; atrophy (thinning) is when it loses elasticity and becomes hard, which is seen a lot in those who live in the countryside due to the cumulative effect of the sun.

“With these techniques we open awareness, now there are 40 people, but social work is beginning, as they begin to transmit their experiences and it is a way to prevent skin cancers and precancerous lesions, such as keratoses.

“The techniques of Cosmetology integrated into Dermatology give a different vision of this aspect and of aesthetic medicine in relation to that specialty, because aesthetic medicine has always been seen as something cosmetic and even superficial, and it is not like that.

"Aesthetic medicine is not only beauty, but it is used in conditions that intervene in health and we have as an example psoriasis, keratosis, among others, and we found a therapeutic result".
