CAMAGÜEY.- When issues related to the Maternal and Child Care Program (PAMI, for its acronym in Spanish) are addressed, the purposes are always superior in favor of the quality of life of children, mothers and women. Dr. Ivette Alicia Prince Martínez, head of her section at the provincial level, addressed the strategies to follow during 2021, always for her, according to her own words: “Do better what went well and do well what went wrong".

She assured that this year they will be directed to everything related to the control of preconception reproductive risk, as it is the base of the pyramid when maintaining not only adequate indicators in the Program, but also a good quality of life for children and women.

The also specialist of 2nd. degree in Pediatrics and Intensive Care and Emergencies, Assistant Professor and Master in Infectious Diseases, said: “Preconception reproductive risk is part of the control in adolescent pregnancy and we must ensure that in this age group they know even the smallest detail of reproductive health, something that concerns both males and females ”.

For this purpose there are joint skills with the Ministry of Education to bring contraception as close as possible to the school, educating parents and adolescents in this regard.

Among other dangers at that stage of life, she announced: “You have to know how to deal with the chronic diseases you suffer from, hence the need to visit your family doctor, and once your condition is diagnosed, be followed by your doctor bedside to guarantee a control that gives rise to know if she has the possibility of conceiving a child because there are some that invalidate the pregnancy due to the risk that fertilization would imply for her. In these cases, definitive contraception should be indicated, while if it does not prevent you from conceiving your child you should receive guidance at the right time to achieve it. "

It is worth knowing that all women when planning their pregnancy must maintain adequate nutrition and vitamin levels, stable hemoglobin figures and a significant amount of folic acid, since it has been proven that this alone reduces 50% the number of possibilities of congenital malformations and brings added benefits for maternal and fetal health.

When approached about the most worrying chronic diseases in this stage of life, Dr. Ivette stated: “Obesity  -pregnancy, which in fact is a health problem in Camagüey, since we find between 23 or 25% in the reproductive stage, undoubtedly very high and carries risks for the mother, such as high blood pressure http: // www. adelante.cu/index.php/es/consultas-medicas/9068-las-pregnancy-care-con-la-hypertension-arterial, diabetes, various metabolic disorders that affect the pregnant woman and the fetus, and other complications. The child has the risk of being macrosomic with all the dangers of being a large fetus or, on the contrary, being one with delayed intrauterine growth (CIUR, for its acronym in Spanish), that is, very small.

"For all of the above, she said, if she is obese, those interested in giving birth have to try to lose weight before conceiving their baby, and also stabilize their metabolism, such as: triglycerides, cholesterol, blood glucose, when the latter is high it can cause malformations in the child ”.

It stands out that each pregnant woman has a personalized control according to the medical specialty that she requires because complications from other causes, and not because of her condition itself, are frequent, was one of the observations of Dr. Ivette,

And she added: “All without overlooking the reorganization of services and the constant training of human resources and that they are available in the event of any obstetric complication. The PAMI does not cease its functions, neither by schedule nor day of the week ”.

When we were interested in learning about other people involved in this Program, the specialist stressed that Health workers have great responsibilities; however, she pointed out that this is a program that includes people, it is women who become pregnant, their partner will be the father of the child and the family awaits the arrival of a new member, that is why everyone must prepare .

"The girl and the adolescent must have a family that cares about teaching them the details, seeking help, if necessary, and going to specialized consultations at the level of health areas, with their obstetrician, without forgetting that there is a highly recommended consultation Qualified in the pediatric hospital Eduardo Agramonte Piña, destined for them with a view to guaranteeing reproductive health ”.

When inquiring about her, if everything had failed ?: "Then the girl becomes pregnant, she explained, and the family also plays a very important role, as they do not present the psychological or biological maturity necessary to face their condition and that is one of the reasons why they commit indiscipline regarding adherence to medical treatment and even escape from Health institutions , and we even stumble upon incorrect decisions when persuaded by the boy's family, which in fact becomes gender discrimination for avoiding responsibilities ”.

And she added: "Pregnant women of any age are called to discipline, understand compliance with the treatments and with the complementary tests indicated to the husband for their importance in the three trimesters and are related to sexually transmitted diseases that can affect the fetus and cause a transmission of this congenital type which causes his permanent disability.

"The syndrome of vaginal dischargeis an entity framed in the signs of vaginal discharge during pregnancy and before these are sexually transmitted diseases that include the couple, it can cause spontaneous abortions, preterm delivery or diseases that infect the fetus when passing through the birth canal (at the time of birth), and if this problem is detected, she must strictly comply with medical treatment, both she and her partner "

The expert did not ignore the need to diagnose and treat urological conditions because urinary tract infections in the pregnancy stage are dangerous for the mother because they are associated with generalized infection, they lead her not infrequently to a delivery before the time required and put the offspring at risk. In situations like this, they are admitted to the maternal homes if she gives positive urine even though they have no symptoms.

Ivette Alicia made a stop in the health of the father and affirmed: “It matters because genetics are worth a lot. The father's metabolism is essential and the consumption of folic acid classifies in the same way in this case because it avoids some congenital malformations. It is impossible to expect satisfactory results without the participation of each and every individual sees his own health as important.

"To successfully achieve the above we know that in Primary Health Care (PHC) many of these difficulties can be solved because in prevention we find the greatest achievement and the hospital is left to respond for a safe delivery."
